Cell and gene therapy has crossed from experimental to clinical. Multiple modalities are approved. The constraint now is not the biology. It is the system that delivers the biology to the patient.

The system
is not ready.
Release at speed, the vein to vein chain and reimbursement remain unsolved at the scale the field demands. Europe now assesses the clinical case for advanced therapies jointly, while price and access are still decided market by market. The delay costs lives.

The principals
are the format.
These problems are not solved by working groups, white papers or conference panels. They are solved when the people with the authority to decide sit in one closed room, by invitation, with no broadcast and no performative Q&A. The room is the mechanism.
